Capturing Social Determinants of Health Using Z Codes

Selecting Z Codes to Capture SDOH

We’ve discussed 10 Z code categories, Z55-Z65, that reflect socioeconomic and psychosocial influences on patient health.

We’ll now review scenarios related to these categories and discuss codes that capture more specific aspects of SDOH.

These scenarios are only intended as examples for your consideration. For each scenario, we indicate which codes may be relevant based on the information presented. But you’ll need more information before assigning a code.

Remember that diagnostic codes become part of a patient’s electronic health record and can appear on after-visit summaries and explanations of benefits. SDOH Z codes that involve sensitive topics, like codes related to interpersonal violence or domestic abuse, need particular attention.

Before assigning Z codes in a real-life situation, ask questions to make sure they apply and thoughtfully consider their use. Pay close attention to sensitive situations and avoid adding codes after a visit without the patient’s explicit consent for inclusion in their medical record.
