During a visit with a new therapist, Jason talks about 2 issues.
Jason mentions a ringing in his ears when he leaves work at a warehouse.
Jason says he operates a forklift. He complains about the forklift’s engine noise and an alarm that beeps when he reverses. He says he often uses hearing protection, but not always. His therapist documents this for coding later.
A medical assistant asks Jason to fill out a questionnaire during his appointment. One of the questions is “Are you worried about losing your housing?” Jason answers yes.
Jason’s new therapist asks him about his response. Jason says his family lives in low-income housing. They asked the landlord for months to fix the broken heating system.
He worries the landlord may evict him and his family. The therapist documents this.
