Clinical Quality Measures 487 & 498

The 2024 PFS final rule also established 2 clinical quality measures – Q487 and Q498 – that relate directly to SDOH and can affect CMS payment.

If a clinician uses Q498 or Q487 to calculate their performance score, then the clinician may get a payment adjustment under the Merit-based Incentive Payment System (MIPS).

The 2024 PFS final rule established expanded-use clinical quality measure Q487 that is now available in 16 MIPS Value Pathways (MVPs) models, 5 of which are new.

Q487 reports the percent of patients 18 years and older screened for several SDOH, including:

  • Food insecurity
  • Housing instability
  • Transportation needs
  • Utility difficulties
  • Interpersonal safety

Q498 works with Q487 and reports the percentage of patients 18 years and older who contacted an organization like a nonprofit or a state or local agency that addressed at least 1 social need within 60 days of completing screening.
