During the start-of-care visit, the home health physical therapist (PT) reviews Bill’s list of medications and notices he’s diabetic and takes insulin. After asking Bill to walk to the refrigerator to get his medication as part of a functional assessment, the PT notices that:
- Bill doesn’t have a lot of food in his refrigerator
- The refrigerator isn’t running
Bill says he can’t pay his power bill and he lives on Supplemental Nutrition Assistance Program (SNAP) benefits.
