Capturing Social Determinants of Health Using Z Codes

Video Transcript: Meet Jason

Next, we meet Jason.

Jason is 26 years old and lives with bipolar depression.

He tells his provider he recently aged out of his parents’ insurance coverage. He makes too much money at a warehousing job to qualify for subsidized, publicly available insurance.

He had a great relationship with a therapist covered by his old insurance but is now struggling to find a therapist he can afford.

He says he lives with his parents because he can’t afford to live on his own, but they’re worried about being evicted.

Based on Jason’s experiences, his provider sees issues involving social determinants of health in a couple of areas.

Because Jason is uninsured and struggling to find an affordable therapist, his provider documents potential issues involving Health Care Access and Quality.

Because Jason said he lives with his parents, can’t afford another place to live, and said he’s worried about his parents’ potential eviction, his provider also indicates that issues related to Economic Stability may affect Jason.

Jason needs affordable mental health and insurance support, which can come from a single provider or from an organization that offers health insurance help or affordable health care. He may also need support with housing issues.
