Now, you’ll meet Sam, who has a different life experience.
Sam is a 72-year-old man who’s uninsured.
Sam tells his provider he emigrated from Vietnam more than 40 years ago and speaks English as a second language.
He has Chronic Obstructive Pulmonary Disorder (or COPD), but he hasn’t seen a doctor regularly because he doesn’t have health insurance. He also prefers a Vietnamese physician but can’t find 1 where he lives.
Sam lives with family, including his son, who is a low-wage, essential worker.
He tells his provider he relies on his son and family for transportation and other support because he doesn’t have any income of his own.
Sam’s provider notes social determinants of health that affect Sam in at least 3 areas.
Because Sam speaks English as a second language and is uninsured, his provider has concerns related to Health Care Access and Quality.
Because Sam doesn’t have his own transportation, his provider also sees issues related to Neighborhood and Built Environment.
Lastly, because Sam has no income and relies on his son, his provider also sees issues involving Economic Stability.
Sam needs affordable health insurance support, access to culturally and linguistically appropriate health care services, better transportation options, and - potentially - economic support.
